At PAN, we understand that our Nurses and Caregivers have the important role of delivering the highest standard of quality care to our Clients providing each and every one with a positive and beneficial experience.  Therefore, we only select individuals who have a genuine passion and desire to provide compassionate home care to join our team. Our Nurses and Caregivers undergo a thorough hiring and orientation process, receive monthly training guides and participate in quarterly in-services that strengthen their knowledge and skills within the home care setting and enhances their ability to provide the highest standard of quality care that each Client deserves. Our staff includes Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), Registered Nurses (RNs), and Licensed Practical Nurses (LPNs).

Private Duty Home Care

Personalized and affordable Private Duty Home Care Service provides safety awareness, assistance with the activities of daily living, companionship and much more.

Geriatric Care Management

Geriatric Care Management is a special and personalized nursing service designed to provide customized care for individuals. It is easy to think of this as “Your daughter for hire.” One of our expert healthcare professionals (all Registered Nurses) will work with you to customize a care plan centered around your specific needs or those of your loved one.

Alzheimer Care

Knowledgeable and experienced Nurses and Caregivers are ready to provide in home care for individuals with dementia, Alzheimer’s disease, or cognitive decline. Our staff of CNAs, HHAs, RNs, & LPNs are matched to fit your requirements.

OTHER SERVICES

Personal Care and Companionship

PAN offers both companionship and personal care services. This includes assistance with activities of daily living such as bathing, dressing, meal preparation and promoting safety.  Our experienced caregivers are there to promote independence and make the everyday activities that have become more difficult a little easier. We understand the importance of social interaction to maintain happiness and decrease depression. We try to incorporate hobbies and social activities into the lives of each Client we serve, as much as possible, as part of the daily care plan.  You can rest assured that our Registered Nurses and scheduling supervisors will be very involved to ensure you are completely satisfied with the services being provided. PAN will also perform periodic assessments to update care plans as needs change.

Respite Care

PAN offers respite care that enables you or your loved one to remain in the comfort of your home, while giving primary family Caregivers time for themselves. We can provide services at home, in hospitals, retirement and assisted living communities and skilled nursing and rehabilitation centers.

End-of-Life Support Care

PAN is here to work with hospice to provide comfort and support to you and your loved one during this difficult time. We can provide a knowledgeable and experienced Caregiver or Nurse to provide the personal and emotional care needed during this difficult time.

PENNSYLVANIA AGENCY OF NURSES INITIAL ASSESSMENT

When the decision is made that you or a loved one are in need of our services. PAN will have a Registered Nurse complete a detailed assessment and develop a care plan. We want you to be comfortable while discussing your needs with a professional. The initial assessment can help you understand the array of services PAN offers, discuss your options and help you understand the true benefits of home care while identifying your specific goals and expectations. The initial assessment will give everyone involved the opportunity to determine what is best for you, your loved ones and any one responsible for planning care at home.
