Care management is a customized consultation and planning relationship with one of our specially qualified professionals. Geriatric care refers to the day-to-day care relationship our caregivers have with the elderly individuals they care for.

Total Medical Coordination

We can help you find the best location for your loved ones (whether that be a personal residence, nursing home, or hospice). We can facilitate communication and will act as a single point of contact for coordination between concerned parties (family, facility staff, and physicians).

Medication Management

Your Registered Nurse Geriatric Care Manager, per your Doctor's orders, will make certain the correct type of medications are in your pill box. We will review your medications with your Physician. We will observe you for any changes or reactions to your medications and will report back to your Physician at once.

Crisis Intervention

We are available to assist individuals who are seeking help, support, and information. Crisis can affect anyone at any time. We are available to provide emotional support and/or referral assistance to meet the needs of your loved ones.

Quick Response

Personal Care & Grooming

Assist client with bathing, showering, dressing, toileting, getting in and out of bed, walking up and down the stairs or around the house, taking a walk and/or doing light exercise.

Meal Preparation

We help prepare meals, including preparation of special diets, assistance with feeding, washing dishes, and cleaning the kitchen after a meal.

Medication Reminders

Pennsylvania Agency of Nurses staff will remind and assist the client in taking their medication on time from the pillbox.

Grocery Shopping

We can help with shopping, food selection, and transportation to and from both the grocery store and other routine locations for miscellaneous errands.

Transportation To Appointments

We can drive clients to appointments with the doctor, to the store, or on routine errands with which they need assistance.

Do We Need A Nurse Or Not? 

Here are three questions to help your family figure out the right qualifications for your situation:

1. Does a loved one need assistance with activities of daily living (ADLs) like toileting, dressing, eating, etc? A CNA or HHA is probably most appropriate.

2. Does a loved one require more specialized care, like catheterization or wound dressing? Some special needs cases require an LPN in addition to CNAs or HHAs.

3. Is a loved one transitioning between different types of care? Our Geriatric Care Managers are best suited for this type of work.
